If you do not have Microsoft Excel on your computer, see if someone else in your office has it and use their computer to fill it out. There are instructions at the top of the document, so please make sure you read these before you try to fill in the form. We have been getting questions from people about things that are answered in the instructions. You should only have to do this once, so read it thoroughly and take your time. As long as you read the instructions, there should be no questions on how to fill it out.
